Member and Guest Waiver Forms

All 18+ adult members and adult guests must have a signed waiver on file to be able to use the pool. Parents/guardians must sign for minor children. If you are bringing a child as a guest, the child’s parent/guardian must submit a waiver. The member hosting the child as a guest may not sign a waiver for that minor.

 

You may also request to sign a paper copy of the waiver at the pool.
